- Name (First and last)
- Social Security number (SSN) or Tax Identification Number (TIN)
- Address of primary residence
- Date of birth
- Valid driver’s license, U.S. passport or other identifying documents
- U.S. bank account number and routing number (cash cannot be accepted)
